How do go about prioritising the different clinical services? | Think about prioritising tasks that have been shown to have a major benefit to patients (med rec, interventions for med related issues, assessment of med orders, clinical review, tdm, adr management, provision of meds) |
What does prioritisation look like day to day? | Identify patients most at risk of medication related problems (who will get the most help from a pharmacy service) Choose the appropriate range of services to provide to these patients (med rec, counselling, supply, tdm) |
In terms of risk, what needs to be considered from the patient perspective? | Age, living status (from home or nursing home facility), cognition, dosage aids |
What are the medication considerations to consider when determining risk? | High risk meds (APINCH) Time critical meds (antiepileptics, parkinsons etc), specific administration instructions, not stocked on the ward |
What are the considerations for medical conditions in determining risk? | What are the expectations for that condition, is this condition time critical, is the admission medication related to their hospital admission, renal and liver function |
List some scenarios where patients are most at risk of a medication related problem | Med related admission, 5+ meds, 12+ doses in a day, med that requires monitoring, on a high risk med, significant change recently, suboptimal control/treatment with meds, difficulty managing, impaired renal/liver function, poor compliance, drs x, hosp rec |
List the time critical medications | Antiarrhythmics, antocaogs, antiepileptics, antineoplastics, corticosteroids, antiretrovirals, drugs for diabetes, immhno-supressants, opioid analgesics, anti parkinsons, reversal agents |
