Week 8 Pharmacology

Week 8 Pharmacology Determining the Rate of Infusion of a Titrated Medication

QuestionAnswer
What equipment is needed to accurately administer critical care medications? infusion pump
Which actions by the nurse are appropriate when titrating a prescribed intravenous medication? 1. Holding the dose 2. Increasing the dose 3. Decreasing the dose 4. Maintaining the dose
Which are examples of vasopressor medications that are titrated? 1. Dopamine 2. Dobutamine 3. Norepinephrine
For the diabetes mellitus patient described in the case study, the nurse should initiate an insulin infusion of mL/hr. 50/100 x 0.4/1 = 40/50 =0.8mL/hr
The nurse initiated insulin infusion for the diabetes mellitus patient at the flow rate of 0.8 mL/hr. The nurse monitors the patient’s blood glucose level one hour later and finds it to be 230. Per the titration order, the nurse will increase the insulin 100mL/50 units x 0.8 units = 1.6 mL/hr
Two hours after initiating the infusion and one hour after titrating the insulin dose, the patient’s blood glucose level is 205. Per the titration order, the nurse increases the insulin by an additional 0.4 units/hour. So the new flow rate is mL/hr. 100 mL/50 units x 1.2 (= 0.4 units+0.8 units/hr) = 2.4 mL/hr
The nurse is titrating a medication for a critical care patient units . When calculating a new flow rate, the nurse calculates the patient should receive 23.45 mL per hour. The nurse proceeds to set the infusion at mL/hr. 23.45 = 23.5 round to the tenths place
The patient is prescribed a renal dose of dopamine 2 mcg/kg/min. The order states that the dopamine should be titrated to maintain a mean arterial pressure (MAP) above 65 mm Hg and below 95 mm Hg. The patient’s current MAP is 60 mm Hg. Which action by the Increasing the dosage until the MAP is within the parameters
The patient is prescribed a renal dose of dopamine 2 mcg/kg/min. The order states that the dopamine should be titrated to maintain a mean arterial pressure (MAP) above 65 mm Hg and below 95 mm Hg. The patient’s current MAP is 80 mm Hg. Which action by the Maintaining the current dopamine dose
Which examples are of anticoagulant medications that are titrated? 1. tPA 2. Heparin sodium
Use dimensional analysis to solve the following. Consider this order: heparin drip: 25,000 units in 500 mL D5W, bolus: 80 units/kg, starting dose: 18 units/kg/hr, patient weight: 70 kg. For a starting dose, this patient should be given ____ mL/hr. 500mL / 25,000 units x 18 units x 70 kg = 25.2 mL/hr
Use dimensional analysis to solve the following. Consider a patient receiving heparin 18 units/kg/hr. The patient’s current weight is 70 kg. The concentration of the current heparin infusion is 25,000 units in 500 mL D5W. This patient’s aPTT is 45 seconds 500mL / 25,000 units x 20 units/kg/hr x 70 kg= 28 mL/hr
Use dimensional analysis to solve the following. Consider a patient receiving heparin 18 units/kg/hr. The patient’s current weight is 70 kg. The concentration of the current heparin infusion is 25,000 units in 500 mL D5W. The current aPTT of this patient 500 mL /25,000 units x 17 units/kg/hr x 70 kg = 23.8 mL/hr
