CKD drugs

QuestionAnswer
sevelamer Renagel, Renvela. TC: Dietary PO4 binder. Reduces serum PO4 in CKD pts on hemodialysis. MoA: Binds PO4 within intestinal lumen, limits absorp and decr'ing serum PO4 conc's without altering Ca, Al, or bicarb conc's. Cat C.
lanthanum carbonate Fosrenol. TC: PO4 binder. Reduces serum PO4 in Stage 5 CKD. MoA: Becomes La+++ in the upper GI & binds dietary phosphate resulting in a net decrease in serum phosphate & Ca++. Cat C.
paracalcitol Zemplar. TC: Vit D analog. Prevent, treat hyperPTHism in CKD. Activates VDR in kidney, lowers serum PTH, improves PTH and Ca homeostasis. Cat C.
doxercalciferol Hectorol. TC: Vit D analog. Prevent, treat hyperPTHism in CKD. Metabolized to active form of Vit D, which balances PTH and Ca levels. Cat B.
calcitriol Calcijex, Rocaltrol, Vectical. TC: Vit D analog. For hypocalcemia, hyperPTHism in CKD. Potent metabolite of Vit D. Cat C.
cinacalcet Sensipar. TC: Calcimimetic. Tx of hyperPTHism in CKD or hypercalcemia in parathyroid carcinoma. Incr's sensitivity of the Ca-sensing receptor on parathyroid gland, lowers PTH and serum Ca. Cat C.
