Thyroid replacement Levothyroxine
Levothyroxine: Action synthetic form of T4 and T3 that increase metabolic rate (protein synthesis, cardiac output, renal perfusion, oxygen use, body temperature, blood volume and growth process)
Levothyroxine: Side/Adverse Effects -overmedication can result in hyperthyroidism -anxiety -tachycardia -palpations -weight loss -menstrual irregularities -insomnia -dysrhythmias
Levothyroxine: Contraindications/Interactions -can increase anticoagulant effects of Warfarin by breaking down vitamin K -antacids, iron/calcium supplements reduce absorption
Levothyroxine: Nursing Considerations/Interventions -monitor PT and INR -decrease Warfarin if needed -monitor s/s of bleeding (bruising, petachia) -educate patient is life long treatment; can't stop use
Antithyroid Methimazole Propylthiouracil (PTU)
Antithyroid: Action blocks synthesis of thyroid hormones and conversion of T4 into T3; prevents oxidation of iodine; treats hyperthyroidism, Graves disease, and thyrotoxic crisis
Antithyroid: Side/Adverse Effects -overmedication can result in s/s of hypothyroidism -drowsiness -weight gain -bradycardia -anorexia -cold intolerance
Antithyroid: Contraindications/Interactions -concurrent use with anticoagulant may increase anticoagulation -concurrent use with Digoxin may increase glycoside levels
Antithyroid: Nursing Considerations/Interventions -monitor for fever and sore throat -liver injury: monitor for jaundice, dark urine, light colored stools and elevated liver function tests -check CBC and hepatic function -give with meals
Radiopharmaceuticals Radioactive Iodine
Radioactive Iodine: Action absorbed by the thyroid and destroys some of the thyroid hormone producing cells; used for high levels of hyperthyroidism and lack of response to other treatments
Radioactive Iodine: Side/Adverse Effects -radiation sickness -feeling of fullness in neck -metallic taste -leukemia
Radioactive Iodine: Contraindications/Interactions -pregnancy (category X) -lactation -inappropriate treatment in young children
Radioactive Iodine: Nursing Considerations/Interventions -allow 2-3 months for full effects of medication -fast overnight before administration -urine, saliva, vomit radioactive for 3 days after use -use full radiation precautions -encourage fluids
