clopidogrel Brand: Plavix TC: Antiplatelet agent (MoA: Metabolite irreversibly blocks P2Y12 on ADP receptors on platelet surface, keeping GPIIb/IIIa complex inactive and blocking aggregation). Ind: Reduce rate of atherosclerotic events if MI/stroke/angina.B.
aspirin Brand: Bufferin, Bayer, Ecotrin TC: Salicylate, antiplatelet agent (MoA: Irreversible COX 1&2 binding, inhibits prostaglandin formation). Ind:(NSAID stuff), prevent/tx MI/stroke. C/D(3rd).
enoxaparin Brand: Lovenox TC:LMWH (MoA: Enhances inhibition rate of clotting proteases- anti-antithrombin III and anti-Xa). Ind: Acute coronary syndromes, DVD prevent/tx. B. BBW- neuraxial anesthesia.
fondaparinux Brand: Arixtra TC: Factor Xa inhibitor (MoA: antithrombin II-mediated, selective Xa inhibition). Ind: DVT and PE prevent, DVT tx. B. BBW- neuraxial anesthesia.
isosorbide dinitrate Brand: Isordil TC: Vasodilator (MoA: Stimulates cGMP, causes VSM relaxation). Ind: Angina, CHF, esophageal spasm w/ GE reflux. C. Note- TID, and need nitrate withdrawal pd btn hs and am doses (~8 hrs).
hydralazine/isosorbide dinitrate Brand: BiDil TC: Vasodilator (MoA: cGMP-> VSM relax AND direct arteriolar vasodilation by hydralazine). Ind: HF. C. Note- No nitrate-free interval needed b/c of hydralazine. $$$
ranolazine Brand: Ranexa ER TC: Misc antianginal agent (unknown MoA). Ind: Chronic angina. C. Note- SE is QTC prolongation, can cause Torsades. 3A4 interxn.
amiodarone Brand: Cordarone, Pacerone. TC: Class III antiarrhythmic- CHEAP! (MoA: Inhib adrenergic stim by a & b-block. Prolong AP & refractory pd in myocardia, decr AV conduction/SA node fx). Ind: V-fib, V-tach. D. BBWs- Liver tox, cause arrhythmias, start in hosp
dofetilide Brand: Tikosyn TC: Class III antiarrhythmic- $$$ (MoA: Increases monophasic AP duration, delays repolarization). Ind: Chronic a-fib/flutter. C. BBW- arrhythmia, so hosp when starting. Note- Major QTC prolongation risk.
