Statins

HMG - CoA Reductase Inhibitors

TermDefinition
Statins Newest antilipemic agents, most potent. Reduces liver cholesterol by preventing conversion of cholesterol in the liver. Increase removal of LDL cholesterol.
Other benefits of statins Decrease inflammation, platelet aggregation, thrombin formation and plasma viscosity.
Need to know Most effective if given at night b/c peak time of production of cholesterol.
Pre med assessment Serum triglyceride and CHO levels, liver function ALT and AST confirm female clients pregnant or trying to get pregnant.
Examples atorvastatin- Lipitor/ fluvastatin- Lescol, Lescol XL / lovastatin- Mevacor/ pravastatin- Pravachol / simvastatin- Zocor
Admin Lovastatin- Mevacor should be given with evening meal to enhance absorption. Others can be given w/o food.
Common adverse effects Headaches, nausea, abdominal bloating, gas
Serious adverse effects Hepatoxicity, Myopathy, Rhabdomyolysis
Myopathy (Chinese ancestry more susceptible) muscle tenderness, pain
Rhabdomyolysis kidney damage resulting from progressing myopathy (may have pinkish - or red tinged urine.
