To prevent the formation of a clot.

Anticoagulant | Used to prevent the formation of a clot and to prevent DVT in venous system. |
What are anticoagulants used for? | MI, Prevent thrombus, and AFIB. |
Oral anticoagulant | Coumadin and Warfarin. |
IV anticoagulant | Heparin |
SQ anticoagulant | Lovenox- Given after surgery. |
Antiplatlet | Prevents thrombus in arteries. |
Antiplatlet Meds | Aspirin and Plavix. |
Thrombolytic | Dissolves clots and re-opens vessel. |
Adverse Reactions of anticoagulants | BLEEDING- INTERNAL/EXTERNAL. N/V, Abd. cramping, diarrhea NO NSAIDS/Asprin NO Thrombolytic w/ Active bleeding |
Nursing Interventions | Assess for bleeding - Nose, gums, stool, and Hold pressure longer. Assess: PT/INR - Coumadin level / 2-3 on coumadin, >5 DANGEROUS, <1 INEFFECTIVE - Stroke out |
