beta lactams

pharm of beta lactams

mechanisms of resistance to PCN inactivation by Beta-lactamase, modification of target PBP's, impaired penetration of drug to target PBP's, presence of an efflux pump
carbapenems indicated for infections caused by susceptible organisms that are resistant to toher drugs and for threatment of mixed aerobic and anaerobic; and enterobacoter infections
Vancomycin active only against gram-positive bacteria ( but also flavobacterium)
Vancomycin administered orally only for C. diff
Vancomycin excreted by kidney, mainly used for spesis or endocarditis caused by MRSA
Monobactams Aztreonam
Monobactams relatively resistant to Beta lactamses
Monobactams active against gram negative rods, no activit against gram positive or anaerobes
Monobactams PCN-allergic patients tolerate without reaction
beta lactamase inhibitors resemble beta lactame molecules--> clavulanic acid, sulbactam, tazobactam
3rd generation cephalosporins extended gam negative coverage, cross the blood brain barrier, active against beta lactamase producing hemophilus and neiseria, used as theray for sepsis of unkown cause
4th generation cephalosporins used for enterobacter
2nd generation cephalosporins better gram negative coverage including Klebsiella but less effective gram positive, oral agents active against beta lactamase producing H. influenza or Branhamella cattarrhalis and are used to treat sinusitis, otits, and lower respiratory infections
1st gen. cephalosporins gram positive cocci, especially pnueomococci, streptococci, and staphylococci
1st gen. cephalosporins excreted by the kidney, used for UTI, minor staph infection and cellulitis
cefazolin penetrates tissues well and is the drug of choice for surgical prophylaxis
penicillins greatest activity against gram positive and gram negative cocci, and non beta lactamase producing anaerobes
antistaphlococcal penicillins resistant to staph beta lactamases active against staph and strep but not enterococci, anearobics, and gram negative cocci and rods
PCN rapidly excreted by kindey, normal half life 30min, in renal failure half fife is 19 Hrs
Nafcillin primarily excreted by biliary
oxacillin, dicloxacillin, cloxacillin eliminated by both renal and biliary
Vanco Drug used for MRSA
Vancomycin MOA Blocks peptidoglycan synthesis
Type of resistance found with vancomycin Point mutation
Penicillins active against penicillinase secreting bacteria Methicillin, nafcillin, and dicloxacillin
Amoxicillin Cheap wide spectrum antibiotic DOC of otitis media
Cephalosporins Class of antibiotics that have 10% cross sensitivity with penicillins
Carbenicillin, piperacillin and ticarcillin PCN active against pseudomonas
Antibiotic causing red-man syndrome, and prevention Vancomycin, infusion at a slow rate and antihistamines
