Neuromuskulære blokkere virkningsmekanisme De binder til de nikotine receptorer på den motoriske endeplade og giver muskellammelse ved at hæmme virkningen af Ach.
Neuromuskulære blokkere indikationer Anvendes til muskelafslapning, sprayes f.eks. i trachea før intubering, benyttes til ortopædkirurgi, til øjenoperationer og til at reducere dosis af sedative midler i balanceret anæstesi.
Hvordan virker administration af neostigmin på en motorisk endeplade som er påvirket af hhv. d-tubocurarin og suxametonium? Ved administration samtidig med d-tubocurarin vil Ach kunne have effekt igen og muskellammelsen aftager, hvorimod den persisterer og endda i nogle tilfælde forstærkes ved administration samtidig med suxametonium.
Hvad er forskellen på d-tubocurarin og suxametonium? d-tubocurarin er en non-depolariserende kompetitiv inhibitor, mens suxametonum er en depolariserende non-kompetitiv inhibitor. Derudover så nedbrydes suxametonium af Achkolinesteraser.
Hvad skal man huske når man administrerer suxametonium til heste og grise? De har naturligt en høj koncentration af Achkolinesteraser i plasma og derfor skal der en højere dosis af suxametonium til for at give muskellammelse hos disse end hos andre dyr.
