Positioning

Shoulder, clavicle, AC joints, scapula

TermDefinition
Shoulder AP external rotation CR 1" inferior to coracoid process. Epicondyles parallel to the IR. Collimate to see the SC joint and 1" above shoulder shadow.
Shoulder AP internal rotation CR 1" inferior to coracoid process. Epicondyles perpendicular to the IR. Collimate to see the SC joint and 1" above shoulder shadow.
Shoulder AP neutral rotation CR 1" inferior to coracoid process Epicondyles 45 degrees to the IR. Collimate to see the SC joint and 1" above shoulder shadow.
Shoulder AP oblique projection Grashey method- RPO/LPO. CR 2" medially 2" inferior to superolateral border (feel for the dip). Place palm of hand on abdomen, keep scapula parallel to IR and rotate 35-45 degrees toward affected side to open scapulohumeral joint. Collimate to IR size.
Shoulder Inferosuperior axial projection Lawrence method. Patient supine with arm abducted 90 degrees, epicondyles parallel to floor and use sponges to lift the body ~ 2". CR horizontal and angled 15-30 degrees medially into the axilla out the AC joint. Collimate to IR size or less.
Shoulder Superoinferior axial projection Patient seated leaning over the IR with elbow flexed at 90 degrees and hand in prone position. CR angled 5-15 degrees toward the elbow through AC joint out axilla. Collimate to IR size or less.
Shoulder PA oblique projection Scapular Y- LAO/RAO. CR at medial border of scapula perpendicular to scapulohumeral joint. Rotate patient 45-60 degrees with AC joint and medial border of scapula perpendicular to IR with arm hanging by side. Collimatation= Keep 12" LW bring in 1" CW.
Shoulder Transthoracic lateral projection Lawrence method. CR at level of surgical neck of affected arm on the IR. Unaffected arm raised above head. Collimate to IR size.
AC joints AP projection bilateral CR at level with AC joints and midsternum. Collimate 6x17 CW and use thyroid sheild. Do second exposure with weight and relevel to AC joints.
Clavicle AP projection CR midclavicle. Elevate chin slightly. Collimate 8x12 min 6x12CW, keep SC and AC joints in collimation.
