TNF drugs for RA

| What is Kineret? | Disease Modifying Anti Rheumatic Drug (DMARD), one of the new class called biologic response modifiers. |
| What does Kineret block? | it blocks a protein called Interleukin, a major player in the inflammatory process. By blocking IL-1, Kineret inhibits the inflammatory response in RA including pain. |
| How is Kineret administered? | prefilled 1ml syringes, to be given daily SC |
| How do you store kineret? | This drug must be kept refrigerated until ready to use |
| How does MTX act? | as an Immunosuppressant to calm down an over active immune system and as an anti inflammatory agent. |
| Side effects of MTX? | hepatic fibrosis and cirrhosis, pneumonitis, and an increased risk of infection. |
| Is lymphoma a SE of MTX? | There have been a few studies that show an increase risk of lymphomas in some people taking methotrexate; however, the disease appears to go into remission when the drug is stopped. |
| Plaquenil facts? | the least toxic DMARDs and less costly to monitor |
| Plaquenil SE? | The major toxicity problem is retinal damage, which can lead to visual impairment. This problem is very rare, and can be reversed if caught early. |
| How long must patient take plaquenil for them to see effect? | It can take 3-6 months before the full benefit of the medication is felt. |
| How does Arava work? | works by inhibiting certain enzymes, dihydroorotate dehydrogenase, and has an anti inflammatory response as well. |
| How ia Arava taken? | PO usually 100mgs a day for three days as a loading dose, then 20mg per day thereafter. Most people begin to notice an improvement in as little as a month. |
| SE of Arava? | liver problems, GI disturbances, and hair loss. |
| What is Enbrel | captures the excess TNF before it can go to work causing inflammation. |
| Cost of Enbrel? | around $1000 per month |
| How is Enbrel administered? | Given twice weekly by injection |
| Azulfadine | reduce inflammation and slow disease progression. |
| SE of Azulfadine | aching of joints headache (continuing) itching increased sensitivity of skin to sunlight skin rash |
| Precautions of Azulfadine | It is important that you drink plenty of fluids while taking Azulfidine to avoid kidney stones |
| How long Azulfadine takes effect? | up to 12 weeks for relief to occur. |
| Drug interactions of Azulfadine? | Anticoagulants (eg, warfarin) or methotrexate because the risk of their side effects may be increased by Azulfidine |
| Most common SE Azulfadine? | nausea |
| Azulfadine dosing? | 500mg tablets. Average maintenance dose is 2g (4 tablets) daily |
| Imuran dosing? | tablets are available in 50mg. Dose is based on a persons weight with the maximum dose being 2.5 mg per kg of weight per day. |
| How do you take Imuran? | Stomach upset may be lessened by taking it with food or milk. |
| SE Imuran? | fatigue, nausea, diarrhea, loss of appetite and hair loss. |
| Minocin drug ineractions? | Certain antiacids and calcium supplements may decrease the effectiveness. |
| What does Minocin do? | Unsure how antibiotiv effects inflam. New pts. |
