Thoracic Spine

Projections, positions,Area of interest, best seen criteria for Thoracic Spine

QuestionAnswer
What are the standard views for the Thoracic Spine? AP, Lateral - Obliques if Zygapophyseal joint needs to be shown
What is the area of interest for an AP projection of the thoracic spine? Open intervertebral disk spaces - and transverse processes of all 12 thoracic spine
What is the SID for an AP projection of the thoracic spine? 40" SID
What is the focal spot size for any projection of the thoracic spine? Large focal spot
Where do you center for an AP projection of the thoracic spine? T7 (inferior border of the scapula), center midsagittal plane of body to midline of the grid
What is the direction of the IR for a AP projection of the thoracic spine? 14x17 lengthwise - top border of IR 1.5 - 2inches above pt's shoulders
What is the respiration for an AP projection of the thoracic spine? The patient can take shallow breaths during the exposure, suspend at the end of full expiration
What is the direction of the CR for an AP projection of the thoracic spine? Perpendicular to the IR center halfway between jugular notch and xiphoid process (T7/T8)
What is the best seen criteria for an AP projection of the thoracic spine? All 12 thoracic vertebrae from T1-L1 possible, no rotation as seen by spinous processes at the midline of the vertebral bodies
What is the area of interest for an Lateral projection of the thoracic spine? Intervertebral foramina - open intervertebral disk spaces of all 12 thoracic spine
Where do you center for an Lateral projection of the thoracic spine? Center posterior half of thorax to midline of the grid and at level of T7
What is the direction of the IR for a Lateral projection of the thoracic spine? 14x17 lengthwise - top border of IR 1.5 - 2inches above pt's shoulders
What is the direction of the CR for a Lateral projection of the thoracic spine? Perpendicular to the IR at level of T7 - CR enters through posterior half of the thorax
What do you do if the vertebral column is not elevated to a horizontal plane when the patient is an a lateral recumbent position? You want to angle the CR/X-ray tube so that it follows the direction of the spine, or perpendicular to the long axis of the spine. An average of 15-20 cephalic - Angle of 15 deg for males and an angle of 10deg for females is for average pts
What is the best positioning for a lateral thoracic spine? Patient in left lateral recumbent, a support is under the lower thoracic region to elevate the vertebral column to a horizontal plane. Also a lead shield is placed behind the patient to absorb the scatter radiation. No angulation of CR needed
What projection is used if upper thoracic region is area of interest? Lateral Cervicothoracic - Swimmers technique
What is the SID for a Lateral projection of the Thoracic spine? 40" SID
What is the respiration for a Lateral projection of the thoracic spine? At the end of expiration; or pt breathing normally but increase exposure time to blur lung markings
What is the best seen criteria for a Lateral projection of the thoracic spine? All 12 thoracic vertebrae from T1-L1 possible, intervertebral foramina, open intervertebral disk spaces, no rotation as seen by ribs superimposed posteriorly.
What position is best used to see the thoracic zygapophyseal joints? AP or PA oblique
What position is best used to see the thoracic intervertebral foramina? Lateral
What position is best used to see the thoracic intervertebral disk spaces? AP
What position is best used to see the thoracic zygapophyseal joints? AP or PA oblique
What position is best used to see the thoracic intervertebral foramina? Lateral
What position is best used to see the thoracic intervertebral disk spaces? AP
What position is best used to see the thoracic zygapophyseal joint space of C7-T1? Cervicothoracic - Lateral Swimmwers
What is the degree of body rotation used for an oblique projection of the thoracic spine? Coronal plane forms a 70degree angle from the IR - 20degrees anterior (PA oblique) or 20degrees posterior (AP oblique)
Where is the centering for a oblique projection of the thoracic spine? Center the vertebral column to the midline of the grid or T7 inferior border of the scapula
What is the direction of the IR for an Oblique projection of the thoracic spine? 14x17 lengthwise - top border of the IR 1.5-2inches above pts shoulders
What is the direction of the CR for an oblique projection of the thoracic spine? Perpendicular to the IR entering/exiting through T7 inferior border of the scapula
What is the area of interest for an AP RPO/LPO oblique projection of the thoracic spine? Side up or Side down? AP - SIDE UP area of interest Zygapophyseal joints farthest from IR
What is the area of interest for an PA RAO/LAO oblique projection of the thoracic spine? Side up or Side down? PA - SIDE DOWN area of interest Zygapophyseal joints closest to IR
What is the SID for an oblique projection of the thoracic spine? 40" SID
What is the type of curve on the thoracic spine? Kyphotic Curve
How many vertebrae are in the thoracic spine? 12
What is the landmark of the xiphoid process in relation to the thoracic spine? T9/T10
What is the landmark of the jugular notch in relation to the thoracic spine? T1
