Mercaptopurine MOA purine derivative, cell cycle specific (S- phase)
mercaptopurine indications off label treatment refractory CD and UC
Azathioprine... same as mercaptopurine
Cyclosporine MOA inhibits first phase of T-cell activation
Cyclosporine Side effects high lipids, high TG, hyperkalemia, hyperglycemia, increased SCr and BUN
Cyclosporine indications off-label treatment-refractory CD or steroid dependent UC
Warning signs of IBS (ROME CRITERIA) rectal bleeding, weight loss, iron-deficient anemia, nocturnal symptoms, family history
IBS drug for abdominal pain antimuscarinics (competitive inhibitor at cholinergic receptors, results in decreased mobility)
IBS drugs for diarrhea anti-diarrheals and serotonin antagonists
Loperamide (anti-diarrheal) MOA interferes with peristalsis by direct action on circular and longitudinal muscles of intestinal wall
Diphenoxylate (anti-diarrheal) MOA focus on smooth muscles, chemically related to opioids and has their effect at high doses
Alosetron (serotonin antagonist) MOA selectively blocks GI based seratonin receptors
Alosetron indication/warning women with severe IBS-D that hasn't been taken care of with other drugs.... BLACK BOX WARNING: ischemic colitis and death!!
IBS constipation drugs laxatives, selective chloride channel activators
Lubiprostone (selective chloride channel activator) MOA pushes Cl into lumen through C-2 channels
