HDL high density lipoproteins are smallest and most dense lipoproteins. function to transport cholesterol from peripheral cells to liver, where it is metabolized and excreted.
lovastatin (Mevacor)Treats primary hypercholesterolemia caused by an elevated LDL cholesterol level not controlled by diet or other treatment measures.
Lovastatin Pharmokinetics Exstensive first pass hepatic extraction. Peak reached in 2-4hrs. Initial response seen in 1-2 weeks. dose = 20mg daily with meals. maximum 80 mg daily.

lovastatin Drug interractions Grapefruit juice, cyclosporine, antifungals
Niacin (nicotinic acid, Nicobid) Niacin (vitamin B3) is water-soluble vitamin that can lower total cholesterol and triglyceride levels by inhibiting VLDL-C synthesis and can also increase HDL cholestrol levels.
Niacin Indication treats hyperlipidemia, hypercholesterolemia, and prevent Vit. B3 deficiency.
Niacin Pharmacokinetics Well absorbed orally. half-life of approx. 45 minutes. 1g PO TID,
Niacin Adverse Effects Flushing, red-skin on the face and neck.
Niacin Drug interaction Concurrent use of vasodilators may increase the likelihood of flushing and postural hypotension.
Cholestyramine (Questran)bile acid sequestering agents.
Cholestyramine Pharmacokinetics plasma cholesterol levels usually decrease 1-2 weaks after initiation of therapy.
Cholestyramine Adverse Effects and Drug interaction constipation, indegestion, abdominal pain, N/V. reduces systemic absorption of digoxin, corticosteroids, and warfarin.
Gemfibrozil (lopid) treatment of hyperlipidemia when triglyceride levels are elevated. Inhibits peripheral lipolysis and decrease the hepatic extraction of free fatty acids, which result in reduction of triglyceride production.
Gemfibrozil Adverse Effects muscle aches, gas, vomiting, diarrhea, rash.
Gemfibrozil Drug interactionss increase in anticoagulant effect when used with warfarin. monitor PT/INR
Ezetimibe (Zetia) 2-Azetidinones. inhibits cholesterol absorption at the brush border of the small intestine.
Ezetimibe Adverse effects headache, GI upset
Ezetimibe Drug interactions use of cyclosporine, gemfibrozil, clofibrate, or fenofibrate may increase levels of ezetimibe. dosing with bile sequestrants (cholestyramine) reduces absorption.
Ezetimibe Nursing management risk for an upper respiratory infection.
