Cardiac Failure

Drugs used to treat cardiac failure

Drug nameType and Mechanism of action
Frusemide Loop diuretic. Act on Loop of Henle, increase Na secretion, decrease blood volume and BP.
Chlorothiazide Thiazide diuretic. Inhibit NaCl co-transport, decrease Na reabsorption, decrease blood volume and BP.
Enalapril, Perindopril ACE inhibitor. Inhibit conversion of Ang1 to Ang2. Suppress Ang 2 and Aldosterone production, dilate arteries=decrease after load
Losartan, Candesartan Ang1 receptor antagonist. Block action of Ang2 on vessels=decrease preload and after load, decrease contractions.
Nitrates Vasodilator. Nitirc oxide donors, dilates veins via cGMP, decrease preload
Carvedilol Beta non-selective antagonist. blocks A1=vasodilation, blocks B1= blocks reflex tachycardia, renin release, blocks B2=increase stroke volume.
Digoxin Glycoside, inotropic agent. Increase contractility by inhibiting membrane NaKATPase. Enhances parasympathetic activity=vasodilation, decrease HR.
Milrinone PDE Inhibitor, inotropic agent. Increases contraction by preventing cAMP breakdown
Dobutamine, Dopamine B1 selective agonist. Increases contraction by sympathetic activation.
