Gout, and Systemic Lupus

clinical presentation of gout pain-attack at night or monring; affects ankles, heels, knee, wrist, finger, elbow, instep; may experience fever, chills, malaise, skin becomes red, hot, swollen, tender
primary gout overproducers (>600 mg), underexcreters, enzyme defects
secondary gout (drugs) salicylates, diuretics (except spiranolactone), ethambutol, pyrazinamide, nicotinic acid, alcohol, cyclosporine
acute gout treatment colchicine (within 12-36 hours); NSAIDS; Corticosteroids
Colchicine stop taking if get diarrhea or ab cramps, dont exceed 8 g/day; effective if use within 12-36 hrs
Colchicine ADR nausea, bloating, emesis, diarrhea, BMS, extravasation
Indomethacine effective for acute gout attack at timeany
CS for acute gout attack, given intraarticularly, IV, PO, last line, ACTH
Gout prophylaxis probenecid, sulfinpyrazone, allopurinol
Benemid ADR uric acid stones, abdominal dicomfort; don't give if CrCl<50
Allopurinol ADR rash --> SJS, alopecia, neutropenia, hepatitis
Allopurinol DDI Azathioprine, Amoxicillin, Ampicillin
