| Heparin | MOA: enhances activity of antithrombin 3 (inactivates thrombin-2, factor 13, 9, 10, 11 & 12), used for PE, DVT, MI & those on dialysis, monitor tx by looking @ the aPPT (partial thromboplastin time) in the intrinsic pathway, *Antidote = protamine sulfate* |

| Warfarin | MOA: antagonist of Vit K (- factors 2, 7, 9, 10), monitor tx by looking at the PT in the extrinsic pathway, *Note antidote is Vit K*; ADRs: 99% bound to albumin, easily displaced from albumin, hemorrhage, contraindicated in pregnancy b/c crosses placenta |
