USMLE Step 1

DrugMOA & ADRs
Crystalline zinc insulin (CZI) MOA: 'Regular Insulin', reduces in 30-60 mins, peak 1-2 hrs, duration 7 hrs, used if needed to drop glucose levels soon, short acting or rapid acting insulin; ADRs: hypoglycemia, allergy, fat atrophy
Insulin lispro MOA: rapid acting insulin, onset 15-30 mins, peak 30-90 mins, duration of 3 hrs; ADRs: hypoglycemia, allergy, fat atrophy
Insulin aspart MOA: onset 12 mins, peak 1-3 hrs, duration 3-5 hrs; ADRs: hypoglycemia, allergy, fat atrophy
Lente MOA: intermediate acting insulin (daily tx), onset 2-4 hrs, peak 4-10 hrs, duration 10-16 hrs, mix of semi & ultra; ADRs: hypoglycemia, allergy, fat atrophy
Neutral Protamine Hagedorn (NPH) MOA: intermediate acting insulin (daily tx), similar profile to Lente, i.e. Humulin 70/30; ADRs: hypoglycemia, allergy, fat atrophy
Ultralente MOA: comprised of lrg particles that are slow to dissolve thus slow onset of action and longer duration, long-acting insulin, onset 2-4 hrs, no peak, duration of 24 hrs; ADRs: hypoglycemia, allergy, fat atrophy
Insulin glargine MOA: long-acting insulin, onset 2-4 hrs, no peak, duration of 24 hrs; ADRs: hypoglycemia, allergy, fat atrophy
Protamine zinc insulin (PZI) MOA: long-acting insulin, onset 2-4 hrs, no peak, duration of 24 hrs, CZI treated w/ protamine; ADRs: hypoglycemia, allergy, fat atrophy
EXUBERA (insulin human [rDNA origin]) MOA:inhaled insulin powder,rapid-acting,peak 30-90 mins,duration similar 2 'regular insulin',recombinant DNA preferred b/c less antigenic;*Note 18th Oct '07 Pfizer will no longer be making product available 2 pts*;ADRs:hypoglycemia,allergy,fat atrophy
Semilente MOA: given subcutaneously, rapid onset, peak 2-3 hrs, duration 5-8 hrs; ADRs: hypoglycemia, allergy, fat atrophy
