Binds tubulin and microtube asso prot disrupting assembly of mitotic spindle for skin/hair/nail fungal infx (Trichophyton, Microsporum). SE: Hepatotox, albuminuria. Contra: Preg. Induces P450. Griseofulvin
Inhibit squalene epoxidase (no conversion of squalene to lanosterol) for Onychomycosis, Tinea corporis/cruris/pedis/capitis. SE: Hepatotox, SJ synd, psoriasis, Lupus. Topical Terbinafine, Naftifine, Butenafine
Inhibit 14Alpha-sterol demethylase (no lanosterol to ergosterol) for Coccidiodides immitis, Crytococcus neoformans, Candida, Histoplasma. SE: GI probs, hepat probs, GYNECOMASTIA, dec libido. Contra: do not use w amphotericin B or triazolam. Inhibits P450 Ketoconazole (Imidazole)
Inhibit 14Alpha-sterol demethylase (no lanosterol to ergosterol) for Aspergillosis, blastomycosis, candidiasis, histoplasmosis, onychomycosis. SE: Heaptotox, SJ synd. Contra: Preg. Inhibit P450 Fluconazole(and for cryptococcal meningitis), Itraconazole (Triazoles)
Form pores in ergosterol membrane for life-threat Aspergillosis, Cryptococcosis, histoplasmosis, candidiasis. SE: Renal Tox, Cytokine Storm. Must be IV. Amphotericin B - W Flucytosine for HIV cryptococcal meningitis
Form pores in ergosterol membrane for Mucocutaneous Candidiasis. Topical tx, not absorbed systemically Nystatin
Inhibit synth of Beta-(1,3)-D-glucans->disrupt cell wall for esophageal candidiasis. SE: Pruritis, rash, GI probs, inc liver enzymes. Fungicidal against Candida, Fungistatic against Aspergillus Caspofungin, Micafungin, Anidulafungin
