Diuretic drugs
diuretic drugs
| Class | Drugs | Site of Action | Major Indications | Adverse affects |
|---|---|---|---|---|
| Loop Diuretics (20-25%) | Furosemide, Bumetanide | TAL (NaK2CL inhibitor) | Pulm Edema, Resistant Edema | Ototoxicity, vascular collapse, low Cl, Ca, K (with metabolic alkalosis), braking phenomenon (give Na to get it going again, counterintuitive), increase renin activity |
| Thiazides (5%) | Hydrochlorothiazide | Early Distal convoluted tubule (NaCl co transporter inhibitor | Edema, HTN, CHF | high Ca, low K (w/ metab alkalosis), carbohydrate intolerance, increase renin activity (not bad per se, but opposite of what you're trying to do) |
| Carbonic Anhydrase Inhibitors (1-3%) | Acetazolamide | proximal tubule | Urinary alkalinazation, glaucoma, epilepsy | hypokalemia, metabolic acidosis (HCO3 loss) |
| Potassium-sparing diuretics | Spironolactone | Late distal tubule, collecting duct (Inhibit aldosterone syn) | Ascites, K+sparing effects, CHF | hyperkalemia, gynecomastia, mentrual irregularities, increase renin activity |
| Osmotics diuretics | mannitol, urea, glycerol | Proximal tubule | Cerebral edema | Acute volume overload, low Na+/K+ |
