asthma doses

Dosing for asthma drugs

Levalbuterol (Xopenex) 2 inhalations q 4-6 hours
Ipratroprium (Atrovent) 2 inhalations four times a day. Not indicated in asthma although some use in more severe cases.
Methylprednisolone Dosepak
Prednisone 40-60mg/day for 3-10days
Pulmicort BID
Pulmicort Respules BID
Fluticasone (Flovent) BID
Beclomethasone (Qvar, Vanceril Double Strength) BID
Mometasone (Asmanex) 1 p BID
Triamcinolone (Azamacort) 4 inhalations (300 mcg) BID or 2 inhalations (150 mcg) 3 to 4 times a day. (Max 16 p)
Ciclesonide (Alvesco) One puff qd (*don't need to rinse mouth even though ICS)
Budesonide + Formoterol 80, 160mcg + 4.5mcg
Mometasone + Formoterol (DULERA) 100 mcg/5 mcg (mometasone furoate 100 mcg and formoterol fumarate dihydrate 5 mcg) Inhalation Aerosol 200 mcg/5 mcg (mometasone furoate 200 mcg and formoterol fumarate dihydrate 5 mcg) Inhalation Aerosol
Fluticasone + Salmeterol (Advair) BID dosing DPI: 100, 250, 500mcg + 50mcg MDI: 45, 115, 250mcg + 21 mcg
Budesonide + Formoterol + (Symbicort) 80, 160mcg + 4.5mcg
Montelukast (Singulair) 10mg hs Reduced dose in kids down to 2yo
Zafirlukast (Accolate) 20mg bid Reduced dose in kids down to 7 yo
Zileuton (Zyflo) 300mg-600mg qid No kids (<12)
Zyflo CR two 600 mg extended release tablets two times a day within one hour after the morning and evening meals for a total daily dose of 2400 mg.
Ipratropium (Atrovent) MDI 2-3 puffs q 6h Neb: 0.25mg/ml q 6h
Ipratropium + Albuterol (Combivent) MDI 2-3 puffs q 6h Neb: 0.5mg/3ml q 4-6 hours
Tiotropium (Spiriva) Handihaler One capsule via Handihaler once daily (18 mcg/day)
Omalizumab (Xolair®) SQ every 2-4 weeks
