Digitalis (digoxin)

Class Cardiac glycoside
MOA Inhibits Na+ K+ pump=increaed Na and Ca in the intracellular space=increased cardiac contractility (+inotropic effect)
MOA decreases vagal tone=decreased SA node firing,prolongs conduction through AV=(-chronotropic)
MOA In a-fib and A-flutter it decreases the number of atrial impulses(-chronotropic)
MOA improves CO (decreases baroreceptor response=decreased SNS discharging=decreased BP due to (+inotropic effects)
Indications Controls ventricular rate in A-fib/A-flutter
Indications psvt
Indication CHF
Indication Cardiogenic Shock
Contra V-vib/V-tach
Contra 2nd and 3rd heart block- in the absence of a parcemaker
Contra Digitalis toxicity
Supplied 0.25 mg-1mg in 1-2 ml SIVP
Initial dose 4-6 mcg/kg over 5 min SIVP
2nd and 3rd doses 2-3 mcg/kg
CNS side effects Anorexia,confusion,delerium,headache,cns depression,weak
Cardi SE a-fib-LT,V-fib/V-tach-LT,AV block-LT, brady,hypo
Blood SE thrombocytopenia
GI SE diarreah,hemmoraggic necrosis of the intestine,n/v
