oral orders   must be written down and read back to the prescriber and confirmed by the prescriber that the order is correct.

Dosage formula   D/H x Q= X D-desired dose divided by H- what is on hand multiplied by the quanity or unit of measure = X. Q is always 1 for solid forms of medication (tablets, capsules etc)
