Osha Confined Space

Confined Spaces

Attendant Individual stationed outside permit space and performs duties detailed in employer's permit space program.
Authorized Entrant employee authorized by employer to enter a permit space
Blanking/Blinding Closure of a pipe or duct on both ends with a fastened pipe. Should be leakproof under max pressure.
Confined Space (3 things) #1 Large enough for bodily entry #2 Limited or restricted means of entry or exit. #3 not designed for continuous occupancy
Double Block and Bleed Ball Valve closed on both ends of pipe or duct and drain vent or valve in between opened.
Emergency (confined space) any occurence internal or external of space that endangers entrant.
Engulfment surrounding of a person of liquid or flowable solids that could cause strangulation, crushing or constriction.
Entry Occurs when any body part crosses the plane of entry into a permit space.
Entry Permit Document provided by employer to allow entry into confined space
Entry Supervisor Employer, Crew Chief, Foreman that determines if accetible entry conditions are met. Oversees entry operations and terminates entry when required.
T or F: Entry supervisor cannot be attendent or entrant. False! They can do both duties if properly. (but entrant and attendent cannot be the same person)
Hazardous Atmosphere (confined Spaces) atmosphere that may expose employees to the risk of death, incapacitation, or impairment to self-rescue. Injury or Acute illness.
5 Causes of Hazardous Atmosphere: #1 Flam. Gas/vapor/mist in 10%+ of it lower flam limit LFL #2 Airborne combustible dust exceeding LFL #3 Oxygen concentration <19.5% or >23.5% #4 Concentration of any substance which could cause risk of... #5 anything else that poses immediate danger
Hot Work Permit Employers written permission to allow operations that provide a source of ignition
Immediately Dangerous to Life or Health IDLH any condition that poses an immediate or delayed threat to life or that would cause irreversible adverse health effects or would interfere with entrants ability to escape unaided from space.
Inerting Displacing of the atmosphere in a space by a noncombustible gas to such an extent that the resulting atmosphere is noncombustible (produces an oxygen deficient atmosphere IDLH
Isolation process a space is removed from service & protected against the release of energy and material into the space by: blanking/blinding, misaligning or removing section of pipes, double block&bleed, lockout/tagout, OR block/disconnect mechanical linkages.
Line Breaking Inentional opening of a pipe, line, or duct that is or has been carrying flammable corrosive or toxic material, an inert gas or any fluid at a volume, pressure or temp capable of causing injury.
Non permit confined space Confined space that does not have the atmospheric hazards that could result in death
Oxygen Deficient Atmosphere atmosphere containing less than 19.5% oxygen by volume
Oxygen enriched atmosphere atmosphere containing more than 23.5% oxygen by volume
Permit required confined space program employers overall program for controlling and where appropriate for protecting employees from, permit space hazards and for regulating employee entry into permit spaces
Permit Required Confined Space has one or more of the following 4 characteristics: #1 contains or has the potential to contain a hazardous atmosphere #2 contains a material that has the potential for engulfing an entrant #3 Has a configuration an entrant could be trapped or asphyxiated #4 contains any other serious safety hazard
Permit System Employers written procedure for perparing and issuing permits for entry and for returing the permit space to service following termination of entry
Prohibited Condition any condition in a space that is not allowed by the permit during the period when entry is authorized
rescue service means the personnel designated to rescue employees from permit spaces
Retrieval System equipment used for non-entry rescue of persons from permit spaces.
Testing process by which hazards of space are identified and evaluated. Includes specifying the tests that to be performed in the space.
Who's responsibility is it to evaluate if any spaces are permit required confined spaces? the employer using the decision flow chart in appendix A to section
How shall an employer notify employees of a confined space? danger signs or by any other equally effective means.
If employer decides employees will enter a confined space what initial measures will they take? Have a written and compliant permit space program that is available to the employees and their designees.
Employer need not comply with requirements if: #1 the only hazard is atmospheric #2 continuous forced air ventilation will keep the space safe #3 has monitoring data supporting #1&2. #4 if initial entry is needed to determine conditions, special method to be followed #5 Documented findings
What should be done immediately after opening an entrance cover? The opening shall be promptly guarded by a rail and temporary cover.
What internal atmosphere conditions need to be tested (in order) #1 Oxygen content #2 Flammable gases and vapors #3 Potential Toxic Air Contaminents
When is it okay for a hazardous atmosphere to be present during confined space entry Never. If detected during entry, the employees must immediately exit. The atmospheric hazard must be removed and the space retested and documented before reentry
