P-Cardiac Arrest
| Question | Answer |
|---|---|
| What does attendant do before AED analyze? | Determine patient is in cardiac arrest: Assess breathing and carotid pulse simultaneously for no more than 10 seconds(30-45 hypothermia) No pulse and no breathing, or gasping breathing, begin CPR (delegate AED, BVM, O2 and OPA to partner and Fire) |
| What does partner and fire do before AED analyze? | Get AED on and press analyze. Set up BVM, O2, insert OPA/NPA. |
| What should attendant, partner fire, etc do while the AED is analyzing? | Swap compressors to reduce time off the chest. |
| What should attendant do if the AED says shock advised? | Defibrillation 2 minutes of CPR Analyze - check carotid pulse Repeat |
| What should attendant do if the AED says no shock advised? | Start 2 minutes of CPR Analyze - check carotid pulse Repeat |
| What does attendant do when off the chest and the AED is not analyzing? | Complete Primary Survey, Complete Critical History, look for causes of cardiac arrest |
| When do you stop CPR and AED use? | 1. Return of spontaneous circulation or signs of life 2. Patient Transferred to a higher level of care 3. Directed to Discontinue or transport by EP after consult |
| What should you do after three consecutive shocks and no ROSC or 15 minutes of CPR? | Consult EPOS for transport orders when 3 consecutive NO SHOCK advised or 15 minutes of CPR has elapsed |
| What should you do while the AED is analyzing? | Check carotid pulse |
| What should you do if you see signs of ROSC? | Assess pulse |
| Indications? | Patient in cardiac arrest |
| Contraindications: | Verifiable Do Not Resuscitate (DNR) or No CPR orders and Obvious Death |
| Differences in medical cardiac arrest and trauma cardiac arrest? | Trauma Cardiac Arrest: Ensure all injuries have been stabilized and packaged when possible during CPR. Contact EPOS for transport orders. |
| What are the five obvious signs of death? | 1. Evidence of tissue decomposition, including rigor mortis and/or lividity 2. Decapitation 3. Incineration 4. Trans‐section 5. Open Skull Fracture with exposed brain matter |
| What are treatable causes of cardiac arrest? | Hypothermia Sepsis Cardiac tamponade Thrombosis Tension pneumothorax Hypovolemia Hypoxia |
| What should you do if find a treatable cause of cardiac arrest? | Call EPOS early for transport orders. Don't wait 15 min/3 no-shock advised |
