S.O.A.P is an acronym for the four different types of information documented by health care professionals in a medical note
Subjective how a patient experiences and personally describes the problem, as well as personal and family history
Objective investigative process. patients physical exam, laboratory findings, and imaging studies
Assessment health care provider formulates a logical analysis: diagnosis identification of a problem. list of possibilities (differential diagnosis
Plan course of action consistent with the assessment procedure, medication, or collecting further data
body planes proximal and distal proximal:closer to the center of the body distla: farther away from the center of the body
abbreviations for common health records VS- vital signs T- temperature I/O- intake/output the amount of fluids the patient has taken in (by IV or mouth) and produced (usually just urine output)
abbreviations for symptoms or exam findings SOB- shortness of breath A&O- alert and oriented RRR- regular rate and rythm
different ways to administer medicine PO- (per os) by mouth IM- intramuscular PICC- peripherally inserted in central catheter IV- intravenous
abbreviations associated for medical care (R)- right (L)- left (B)- bilateral (both sides)
