Blood Collection Equipment, Additives, Order of Draw

Antiseptics substances used to prevent sepsis, only reduce the amount of microbes, but does not kill them
Disinfectants chemical substances used to remove or kill microorganisms
Tourniquet A device applied or tied around the arm to restrict blood flow
Gauge A number that gives an indication of the size of the lumen of the needle
ETS or multi sample needles needles that are threaded in the middle and have a point at both ends, screw into tube holders and allow for multiple tubes to be collected at one time
Tube holder clear plastic cylinder used to hold tubes
Evacuated tubes tubes used to collect blood for specimen testing
Syringe transfer device allows safe transfer of blood from syringe to tube without removing the tube stopper
Winged infusion set butterfly needle
Gloves must be changed every procedure, powdered and latex gloves are not approved
Antiseptics are used on humans, most common is isopropyl alcohol
Disinfectants are used to kill or remove microbes, not safe for skin, used to clean instruments and surfaces, bleach is most common
Gauze used following VP, cotton balls not used because they can tear off platelet plugs when removed
Do not put bandages on children younger than 2 years of age
Sharps containers need to be identified with a biohazard symbol, be rigid, puncture resistant, leakproof and disposable with a locking lid
Tourniquets must be applied tight enough to restrict venous flow, but not arterial flow, can be disposable, should
Bariatric tourniquets are to be used for obese patients
Bevel pierces skin
Lumen internal gauge
Gauge indicates lumen
21-gauge are most commonly used
Every needle must have a safety feature
ETS (evacuated tube systems) are the preferred collection system of the CLSI
Vacuum is created in the tube during manufacture, can be lost if tube is expired or dropped
Common anticoagulants (ECHO) EDTA, Citrates, Heparin, Oxalates
EDTA ethylene/diamine/tetra/acetic acid must be mixed immediately to prevent platelet clumping and micro clot formation
Heparin inhibits thrombin
Antiglycolytic agents prevent the breakdown of glucose by the blood cells, if not prevented, glucose can decrease at a rate of 10 mg/dl
Thixotropic gel is present to form a barrier between serum or plasma following centrifugation
Carry over or cross contamination occurs when an additive is transferred by the needle to subsequent tubes, reason for order of draw
