ADCW Adult Day Care Waiver - a program that supports adult day care services for individuals with disabilities.
ADW Aide for Daily Living Waiver - a program that provides funding for personal care aides.
ALF Assisted Living Facility - a residential option for seniors that provides personal care services.
APC Advanced Practice Clinician - a healthcare professional with advanced training and skills.
CDS Consumer Directed Services - a service model that allows individuals to direct and manage their own care services.
DMH Department of Mental Health - the Missouri state agency responsible for mental health, developmental disabilities, and substance use services.
EMOMED Electronic Missouri Medicaid (eMOMED) - Missouri's Medicaid eligibility and claims processing system.
FPL Federal Poverty Level - an income guideline used to determine eligibility for Medicaid and other assistance programs.
FSD Family Support Division - the Missouri division that determines eligibility for MO HealthNet (Medicaid) and other public assistance programs.
GHE Group Home Environment - a residential setting that provides supervision and support for individuals with disabilities.
HCBS Home and Community-Based Services - services that allow eligible individuals to receive care in their homes or communities instead of a nursing facility.
ILW Independent Living Waiver - a program that provides services to help eligible individuals live independently in the community.
InterRAI HC International Resident Assessment Instrument–Home Care - the standardized assessment used to evaluate an individual's functional status and service needs.
PC Personal Care - assistance with Activities of Daily Living provided in the home or community.
PCCP Person-Centered Care Plan - an individualized care plan developed with the participant based on their goals, preferences, and assessed needs.
RCF Residential Care Facility - a licensed residential facility that provides housing and supportive services for individuals who require assistance but not skilled nursing care.
SFCW State Funded Community Waiver - a Missouri-funded program that provides community-based services for eligible individuals with disabilities.
