DEA form 41 (and done) Disposal of controlled substances (due to expiration, damage, or unused) Can dispose by returning to manufacturer, wholesaler, or reverse distributor. Record must be kept for 2 years minimum.
DEA form 106 (hit the bricks) Report loss or theft of controlled substances. Record must be kept for 2 years minimum.
Controlled substance inventory Pharmacy must perform an inventory of controlled substances EVERY 2 YEARS
