Is it OK to substitute medications without a physician's approval? No, you must also check with the doctor.
How many times to do you check the medication against the order before giving it to the patient? 3
Define Parenteral meds. Medications that do not pass through the GI system. Usually refers to injectable meds.
Define topical meds. applied to the top layer of the skin or mucous membranes.
Transdermal patch Patch that contains slow released medication that gives a prescribed dose of medicine over 48-72 hours. Med is absorbed through the skin into the blood stream.
What are some important things to remember when administering a transdermal patch? Use a site that is clean and free from lotions, oils, and hair. Rotate sites to prevent irritation. Wait 30 min. after applying the new patch to remove the old one. Always wear gloves when applying the patch. Write date, time, initials on the patch.
In what ways are inhaled meds given? Nebulizer, metered-dose inhaler.
Where are ophthalmic meds given? Eyes
Where are otic meds given? Ears
Injectable meds can be drawn from: vials, ampules, prefilled cartridges
Define vial. Vacuum-sealed bottle with sterile solution
Define ampule. single-dose glass container with "neck" on the top
Is air injected into a vial? yes
Is air injected into an ampule? no
Define prefilled cartridge unit. contains a single dose of medication for injection. Requires a special holder with plunger.
Define syringes. Tubes made of plastic or glass, coming in a variety of sizes, used to inject medications
Define needle. Sterile metal object made to fit on the end of a syringe.
Gauge refers to: the size of the needle.
The larger the gauge number: the smaller the needle size.
Choosing which needle size and gauge to use is based on: route of administration, patient size, type of medication, injection site, viscosity of meds.
Define intradermal injection placed in the dermal layer of the skin.
What is the purpose of a ID injection? It is a diagnostic medication for TB testing and allergy testing.
What angle do you inject an ID injection? 15 degrees
What is created after giving an ID injection? Bleb or wheal
Where are ID injections given? forearm or hairless areas of the backforearm or hairless areas of the back
What length and gauge needle do you use for an ID injection? 3/8 - 5/8 length, 27 - 28 gauge
Define subcutaneous injection. Placed into the subq tissue.
Where are SQ injections given? upper outer arm, thigh, and abd
How many ml can be given SQ? 1 ml or less
What common medications are given SQ? vaccines, heparin and insulin
Do you aspirate when giving a SQ injection? yes, but not if you are giving vaccines, heparin or insulin
What needle length and gauge is used for a SQ injection? 1/2 - 5/8 inches, 25 - 26 gauge
What angle is a SQ injection given? 45 degrees
Define IM injections. Intramuscular, giving into a large muscle
Name the 4 common sites of IM injections. Deltoid, vastus lateralis, ventrogluteal, dorsogluteal
What angle is an IM injection given? 90 degrees
Do you aspirate before giving an IM injection? yes except with vaccines
What is the landmark for the deltoid IM site? 1-2 inches below the acromion process
How many mls can be given in the deltoid? 1 ml
What is the needle length and gauge of all IM Injection Sites? 1 - 3 inches, 20-23 gauge
Where is the vastus lateralis site? Thigh
Which is the preferred site for infants and young children? Vastus Lateralis
Where is the vastus lateralis injection given in infants and young children? upper third of the thigh
Where is the vastus lateralis injection given in adults? Middle of the thigh
Where is the ventrogluteal site? hip
Where is the dorsogluteal IM site? upper portion of the butt
What are the landmarks for the DG site? Divide the gluteus into 4 quadrants and give into the lateral, superior quadrant (upper, outer).
Which type of injection do you massage the area after administration of the med? IM
How many mls are in a tsp? 5
How many mls are in a T? 15
How many mls in an oz? 30
How many mls in a medicine cup? 30
