Subjective Subject to how a patietn experiences and peresonally descirbes the problem as well as family history
Objective Objective data comprises of labrotory results, physical examination, and imaging
Assessment upon gathering needed information the health care provider formulates logical analysis which identifies a problem, or a list of possibilities.
Plan provider formulates a plan or course of action which could be treatment or furhter testing.
Acute It just started recently or is a sharp severe symptom
chronic It has been going on for awhile now
exacerbation it is getting worse
abrupt all of the sudden
febrile to have a fever
afebrile to not have a fever
