RBT EXAM 2026 PART E- Document & Report

RBT's Role in Communication Team Concerns RBTs act as a bridge between caregivers/instructors and the supervisor. They observe, document, and relay information objectively and promptly while continuing to follow the written treatment plan.
Communication Cycle for RBTs Observe → Document → Relay information to the supervisor → Follow directions from the supervisor. Avoid delaying updates and report information objectively without personal opinions.
RBT Reporting From Caregivers Objective information affecting sessions such as sleep, meals, illness, or schedule changes. Record who reported it, what happened, and when it occurred.
RBTs Seek Clinical Direction from a Supervisor When something unclear, or outside your training. Record facts and contact your supervisor promptly instead of trying to solve the issue alone.
Concerns should be Prioritized when Reporting Safety and Health Concerns such as injuries or medication side effects. These should be reported immediately according to incident or emergency reporting procedures.
RBTs when not Understanding a Proccedure Ask the supervisor for clarification before implementing it. Be specific about what you don't understand to ensure the procedure is implemented corectly.
Data Irregularities an RBT should Report Sudden spikes or drops in the behavior data, missing trials, device errors, or unusual patterns. Report the same day with details like, time, rate, and context.
Chain of Command, if a Supervisor is Unavailable Follow program's backup process, such as contracting a lead RBT or on-call clinician. Document who you contacted and when.
Variables that may Affect Client Progress Illness, sleep changes, medication changes, appetite changes, schedule changes, new providers, or family transitions like travel or moving.
RBTs Report Variable Affecting Sessions Use Objective Language. Include who reported information, what happened, who reported the information, what happened, when it started, and how it may affect the session.
When should Urgent Variable be Reported? Immediately if they affect safety or session feasibility (e.g., fever, serious injury, extreme drowsiness). Follow the program's urgent reporting process.
Session Notes Include Date/time, goals targeted, procedures used, client responses, data summaries, and any barriers encounter during the session.
RBT Documentation be Objective Session notes are part of the client's health record and must be accurate, factual, and compliant with legal and workplace documentation standards.
RBT and Documentation Error Add an addendum or correction according to program rules instead of deleting or altering original records.
Importance about Confidentially in Documentation Client information must remain confidential and stored or shared only through secure systems that follow legal and workplace policies.
