insurance card info

TermDefinition
member ID/ patient ID the number that identifies the policy information including which family members are covered if any
group number the number your employer is given to identify their employee health insurance policy. As an employee, you are a part of that plan.
person code identifies the individual(s) covered under the policy; the policy holder Is usually 00, spouse 01, first child 02, second 03, ect.
pharmacy BIN number sex-digit bank identification number that is necessary to correctly route online claims for payment
