parenteral route syringe: single use, appropriate size Needle: use smallest possible, different sizes and lengths for different areas of body size ranges from 0.5mL to 60mL accuracy maintain sterility while filling

intradermal injection shallow, into dermal layer (just under epidermia)

preparation of vial for injection hand hygiene (gloves), vial containing single dose multiple doses, withdraw med with filter needle of blunt needle (rubber stopped dull needle), remove cap from vial, scrub top with alcohol swab
preparation of medication for injection inject air into vial equal amount of fluid top be withdrawn, invert vial and remove medication, tap syringe to remove air, replace cap (scoop method) and remove filter needle and apply injection needle
preparation of medication from ampule use filter needle to prepare (removes glass), tap top of ampule (make sure its all at the bottom), wrap an alcohol wipe or gauze around neck and snap, can invert the ampule or keep flat to draw up, discard ampule
subcutaneous injections sites abdomen 2 inches away from umbilicus, incisions, stoma, wound lateral aspect of upper arms, thigh, scapula and upper gluteal
sub Q needle sizes, volume, length, and angle 1-3mL, gauge 25-27, length 1/8-5/8 inches insulin syringe for admin of insulin only inject angle 45-90 degree (45 if thin pt, 90 degree if average or obese)
SubQ administration gloves, choose site and cleanse with alcohol, wait to dry, inject, dart like action, slowly inject medication, withdraw needle, engage safety and discard in sharps
IM injection sites ventrogluteal, vastus lateralis, deltoid
ventrogluteal IM site preferred site for adults, safe (away from major blood vessels and nerves), position on side, knee bent Locate greater trochanter and anterior superior iliac spines
vastus lateralis IM site preferred for infants, small children, and some adults, select right size needle, on hand above knee, one hand below greater trochanter, middle third of muscle
deltoid IM site if muscle not well developed- injury risk, can be used for immunizations for adults or older children, small volume 0.5mL, palpate lower end of acromion process, place three fingers below, inject into center of triangle
Z-Track used for IM injection for irritating substances (will decrease pain, irritating, staining at injection site),
Z-Track technique pull skin and SQ tissue laterally, inject at 90 degrees, aspirate with one hand, inject medication and leave needle inserted for 10 seconds, withdraw needle and release skin
intradermal injection shallow injection under epidermis into dermis layer used for TB and allergy testing 1mL syringe, 25-27 gauge, 1/8-5/8 inches sites: forearm and scapula
intradermal injection technique gloves clean skin air dry pull skin and inject needle at 5-15 degree angel, bevel up advance needle 3mm, slowly inject and see bleb (bump in skin) withdraw needle slowly
checks before administering do you have an order? compare to the eMAR. when prepping meds, compare the label to the eMAR 3x. do not give any meds that you did not prepare. listen to pt concerns (usually reveals error)
Assess for non-adherence too many meds, poor pt communication, fear of addiction, perception that med is unnecessary, depression, cognition, func ability, dislike side effects, busy lifestyle, financial
teaching for med administration teach about the medication youre about to give : what is it? why is it prescribed? how long do you have to take it? when? provide easy to read education, schedule, caregiver education, allow questions
calculation needs D= desired dose or order H= on hand or have S= supply X= unknown or answer (amount of drug to give)
fraction type calculation supply/have = X/desire
