Personal Care

Routinely Scheduled Care : Routine care may be delegated to a CNA or to assistive personal. However, as a nurse, it is YOUR responsibility to make sure the care is completed.
Early Morning (Early A.M) Care: Assisting without toileting Give patient a warm, wet washcloth to wash hands & face Assist with oral care if desired Provide fresh water if allowed Do not dim lights
Morning (A.M) Care : Done after breakfast Assisting with a bath toileting oral care back massage, shaving, hair care, and dressing or changing gown. Assist the patient to get out of bed if they can do so. Have a patient sit in the chair while you straighten or change the
Afternoon (P.M) Care: Patient assistance: rest, receive visitors, toileting, making bed, reposition, give fluids(unless contraindicated) (technique or drug should not be used in the case in question
Hygiene: Term used to describe keeping oneself clean and well groomed. Bathing, washing, styling hair, brushing/flossing teeth, dressing, shaving.
ADL: Activities of the Daily Living
Clean The Skin: Remove dirt, bacteria, dead skin cells, sweat and odors. It makes the skin smells fresh and clean to prevent infection.
Patient Preferences and Abilities: When planning personal care and bathing, you need to determine the patient's preferences regarding the time and type of bath.
Timing: If they do not want a bath early in the A.M go back and offer it to them again, being positive and warm wit your approach.
Culture: Some cultures is not permitted to bath a person of opposite sex. plan for same sex care
