Medical History

TermDefinition
Homeostasis The body’s ability to keep a stable internal environment (like temperature, heart rate, and blood pressure) even when the outside environment changes.
Medical History A record of a patient’s past and current health conditions.
Current History Information about what is happening with the patient right now.
Previous History Health problems or treatments the patient had in the past.
Social History Information about lifestyle (home life, habits, stress, etc.).
Family History Health conditions that run in a patient’s family.
Chief Complaint The main reason the patient is seeking medical care.
Physical Signs Changes a provider can see or measure (rash, fever)
Symptoms What the patient feels or reports (pain, nausea).
Diagnosis The name of the illness or condition identified.
Differential Diagnosis A list of possible conditions before the final diagnosis.
Empathy Showing understanding and care for a patient’s feelings.
Demeanor A person’s behavior or attitude.
Tact Being careful and respectful when communicating.
Artery A blood vessel that carries blood away from the heart.
Vein A blood vessel that carries blood toward the heart.
Hyper Prefix meaning “too much” or “above normal.”
Hypo- Prefix meaning “too little” or “below normal.”
-emia Suffix meaning a blood condition
-itis Suffix meaning inflammation.
Chemical Reaction A process where substances change into new substances.
HIPAA A law that protects patient privacy and medical information.
Risk Factor Something that increases the chance of disease.
Triage Sorting patients based on how urgent their condition is.
Immune System The body’s defense system against illness.
Mucus Thick fluid that traps germs and dust.
Lymph Clear fluid that helps fight infection.
Lymph Nodes Small glands that filter lymph and fight infection.
Cancer Disease where cells grow uncontrollably.
Carcinogen A substance that can cause cancer.
Inflammation Swelling and redness caused by injury or infection.
Temperature How hot or cold the body is.
Hyperthermia Abnormally high body temperature.
Hypothermia Abnormally low body temperature.
Thermometer Tool used to measure body temperature.
Heart Rate Number of heartbeats per minute.
Beats per minute (BPM) Unit used to measure heart rate.
Pulse The feeling of blood moving through an artery.
Tachycardia Heart rate that is too fast.
Bradycardia Heart rate that is too slow.
Radial Artery Artery in the wrist used to check pulse.
Carotid Artery Artery in the neck that supplies blood to the brain
Respiratory Rate Number of breaths per minute.
Breaths per minute Unit used to measure breathing rate.
Inspiration Breathing in.
Breathing in. Breathing out.
Blood Oxygen Saturation / Pulse Ox Amount of oxygen in the blood.
Pulse Oximeter Device that measures oxygen levels
Oxygen Gas the body needs to make energy.
Carbon Dioxide Waste gas released when breathing out.
Hemoglobin Protein in blood that carries oxygen.
Erythrocytes Red blood cells.
Blood Pressure Force of blood pushing against artery walls.
Sphygmomanometer Tool used to measure blood pressure.
Systole / Systolic Pressure when the heart contracts.
Diastole / Diastolic Pressure when the heart relaxes.
mmHg Unit used to measure blood pressure.
Korotkoff Sounds Sounds heard when measuring blood pressure.
Brachial Artery Artery in the arm used for blood pressure.
Hypertension High blood pressure
Hypotension Low blood pressure.
Stethoscope A tool used by healthcare providers to listen to heart, lung, and blood flow sounds.
Electrocardiogram (ECG/EKG) A test that records the electrical activity of the heart to check heart rhythm and function.
Body Mass Index (BMI) A number calculated from height and weight that helps estimate whether a person is underweight, healthy weight, overweight, or obese.
