| SOAP Method | An acronym that stands for the four general parts of a medical note: Subjective, Objective, Assessment, and Plan |
| S | It is Subject to how a patient experiences and personally describes the problem as well as family medical histories. |
| O | Objective data comprise the patient's physical exam, lab findings, and imaging studies performed at the visit. |
| A | An Assessment could be a diagnosis, an indication of a problem, or a list of possibilities for the diagnosis. |
| P | The provider formulates a Plan, a course of action with the assessment. It could be a treatment plan with medicine or a procedure. |
| Afebrile | To not have a fever. |
| Auscultation | To Listen. |
| Sequela | A problem resulting from a disease or injury. |
| Etiology | The cause. |
| Proximal | Closer in to the center. |
