TermDefinition
SOAP An acronym for the four parts of a medical note. S = Subjective O= Objective A = Assessment P = Plan
Acute Started recently or is a sharp, severe symptom
Chronic Has been ongoing for a while
Palpation To touch or feel
Oriented The patient is aware of who they are (name/birthday), where they are (city/current location), and what time it is (month, year, etc). A patient who is aware of all three is considered A/O X3
Idiopathic No specific cause is known
Malignant Dangerous or a problem
Palliative Treating the symptoms but not the cause
Prophylaxis Preventative treatment (ex. antibiotics)
Dorsal/Posterior The back (similar to the DORSAL fin on a sharks back)
