TermDefinition
Dehiscence Not a common complication of wound healing, but it is an extremely serious one.
Hemorrhage Surgical wound that may hemorrhage or bleed profusely.
Evisceration Life threatening situation.
Emaciated or Malnourished Emaciation is the state of being very lean or having very little muscle.
Eschar Hard, dry, dead tissue that has leathery appearance. Ex) Brown, Black, Tan
Erythema Indicates the increased capillary blood flow associated with inflammation.
Bruising Discolored areas. Make notations of any such areas that are found so that it will be easy to determine new breakdown.
Debridement Infection treated with antibiotics and surgical removal of the dead tissue.
Methicillin-resistant An extremely serious issue. Ex.) Staph microorganisms, MRSA
Pallor Related to impaired circulation, which is a major risk for skin breakdown.
Sinus Tract A channel or tunnel that develops between two cavities or between two cavities or between an infected cavity and the surface of the skin.
Granulation Tissue A new tissue that is extremely FRAGILE.
Colonized Without signs of infection.
Ampule Small sealed glass drug (Must be broken to withdraw the medication.)
Ischemia Tissues and capillaries are compressed resulting in reduced blood flow. (Delayed wound healing or tissue death)
Infected The infectious process is already established as evidenced by high numbers of microorganisms and either purulent, drainage or necrotic tissue.
Contaminated A surgical wound or a wound caused by trauma that has been grossly contaminated by breaking asepsis.
Wound Closure Sutures, staples, steri-strips, approximation.
Wound Type Open or closed
Seropurulent Both clear drainage and drainage with pus are present.
Bilious Made by the body to help break down fats for digestion. Greenish color and often present in wound drainage after gallbladder surgery.
Serous Drainage becomes clearer to slightly yellow fluid.
Serosanguineous Drainage looks pink.
Clean- contaminated Wound that is surgically made and is NOT infected. Has direct contact with normal flora in the respiratory tract, urinary tract and gastrointestinal tract.
Pressure Injury A wound resulting from pressure and friction.
Laceration An open wound made by the accidental cutting or tearing of tissue.
Abrasion Is a superficial open wound. Ex.) Scrapes, scratches, rub type wounds, carpet burn, skinned knee.
