Why are MAPPs legally required Under the COMAH regulations, high risk sites must have a MAPP to operate legally- chemical plants, fuel storage.

Leading Vs lagging indicators Leading- act as warnings before issues -near-miss, safety training. Lagging - damage reports, showing failures after they occur. leaks/breakdowns etc
Key steps in managing process of risks? ID biggest risk, choose good warning signs to monitor, track what goes wrong, review and improve.
What is a HAZID hazard identification analysis Broad, early-stage brainstorming tool to find out what could go wrong (Hazard)
What is a bowtie analysis A detailed visual method used after ID to show how a specific hazard causes harm. (Threats or Consequence) and what barriers (controls) in place.
Leading Vs Lagging simplified means Leading indicators help prevent incidents Lagging indicators help learn from past failures Regular reviews ensure improvement
