S.O.A.P. S- subjective O-objective A- assessment P- plan an acronym for the four different types of information documented by health care providers in a medical note
MRI magnetic resonance imaging - creates detailed images of soft tissue (muscles, ligaments, brain), bones, and blood vessels
EHR electronic health record - SOAPs, treatments and histories
CBC Complete blood count - provides a look into overall health by measuring RBC, WBC, platelets, hematocrit and hemoglobin
SOB (Dyspnea) shortness of breath

T temperature
F/U follow up
NAD No acute distress (no intense symptoms)
IV intravenous
Lou Gehrig Disease amoytrophic lateral sclerosis (ALS) named after Lou Gehrig who was diagnosed with ALS and raised awareness
eustachian tube the pharyngotympanic tube - connects the middle ear to the nasopharynx and is crucial in pressure equalization and sound transmission
crohn’s disease regional enteritis - type of IBD ; chronic inflammation of the GI tract
Huntingtons disease genetic disorder causing progressive breakdown of nerve cells in the brain - named after George Huntington
fallopian tubes part of the female reproductive system, connecting each ovary to the uterus - named after Gabriele Fallopio
Integumentary system skin, hair follicles, nerves, glands, and nails
skeletal system bones and cartilage
cardiovascular system heart and blood vessels
