Pilocarpine אגוניסט מוסקריני שמעלה רוק ודמעות. משמש לגלאוקומה ויובש בפה. ת״ל: הזעה, ריור, כיווץ אישונים, ירידת ל״ד.
Physostigmine מעכב אצטילכולינאסטרז שחוצה מחסום דם-מוח ולכן מעלה אצטילכולין במוח ובמערכת ההיקפית. משתמשים בו בעיקר להרעלת אטרופין ואנטיהיסטמינים עם השפעה אנטי-כולינרגית. תופעות לוואי כוללות שלשול, הזעה, ריור, כיווץ אישונים וברדיקרדיה — בגלל עודף אצטילכולין.
Donepezil מעכב אצטילכולינאסטרז הניתן בעיקר לטיפול במחלת אלצהיימר כדי לשפר זמנית תפקודי זיכרון וקוגניציה. הוא פועל במוח ומעלה זמינות אצטילכולין בסינפסות. תופעות הלוואי קשורות להפעלה פראסימפתטית: בחילות, שלשול, ירידה במשקל, ברדיקרדיה וסחרחורת.
Atropine אנטגוניסט מוסקריני שחוסם השפעה פראסימפתטית. ניתן בהרעלות של זרחנים אורגניים, בברדיקרדיה סימפטומטית וכדי להרחיב אישונים בבדיקות עיניים. תופעות הלוואי אנטי-כולינרגיות קלאסיות: יובש בפה, עצירות, טכיקרדיה, אישונים מורחבים ובלבול במינונים גבוהים.
Ipratropium אנטגוניסט מוסקריני הניתן בשאיפה לטיפול ב־COPD ואסתמה, וגורם להרחבת סימפונות על ידי חסימת M3 בדרכי הנשימה. הוא כמעט אינו נספג סיסטמית ולכן גורם למעט תופעות לוואי, אך לעיתים מופיעים יובש בפה וגירוי בגרון.
Succinylcholine אגוניסט ניקוטיני הגורם לבלוק דפולריזציה — בתחילה כיווץ שרירים קצר ואז שיתוק מלא, ולכן הוא משמש לאינטובציה מהירה. תופעות הלוואי כוללות היפרקלמיה, ברדיקרדיה, עלייה בלחץ תוך־גולגולתי והסיכון הנדיר אך החמור ל־malignant hyperthermia.
Scopolamine אנטגוניסט מוסקריני בעל חדירות גבוהה למוח ומשמש במיוחד למניעת מחלת ים ובחילות תנועה (מדבקה מאחורי האוזן). תופעות הלוואי דומות לאטרופין: יובש בפה, טשטוש ראייה, עצירות וסדציה, ולעיתים בלבול במבוגרים.
Pancuronium חוסם ניקוטיני לא דפולריזטורי (non-depolarizing) המשמש לשיתוק שרירים במהלך ניתוחים ואינטובציה ממושכת. הוא גורם לחסימה תחרותית של ACh בקולטן הניקוטיני. תופעות הלוואי כוללות טכיקרדיה ועלייה קלה בלחץ דם, והוא מחייב הנשמה מלאכותית עד שהתכשיר מתפנה.
