Amiodarone Cardiac arrest, slows conduction rate. Nausea and vomiting, Bradycardia, Hypotension. no grapefruit
Amlodipine/Nifedipine CCB. For HTN, angina, arrhythmias. Flushing, rash, Arrhythmias, Bradycardia, Edema, Hypotension, Heart failure. Notify HCP if BP < 90/60, Avoid grapefruit
Apixaban Anticoag. Bruising,Increased risk of bleeding, Thrombosis from discontinuation, GI ulcers. Avoid ASA, NSAIDs, antiplatelets Andexanet alfa – antidote
Atorvastatin Manages high cholesterol/CAD. Abdominal Cramping, Diarrhea, Constipation, Skin rash. No grapefruit. Monitor LFT every 6 months, cholesterol, LDL, HDL, triglycerides
Clopidogrel Anti-platelet. Hemorrhage, Bleeding, Thrombocytopenia. Observe for bleeding – gums, bloody/dark stool, bruising, hematuria. No garlic, ginseng or gingko herbs. Stop 5 days prior to surgery
Enalapril/ pril meds ACE, for HTN, CHF, MI. Rash/pruritis, Persistent dry non-productive cough, Orthostatic hypotension, Elevated K+,Take 1h ac or 2 h pc, NSAIDs decrease effects.
Metoprolol Beta Blocker, For HTN, Angina, MI, CHF, arrhythmias. Hypotension , Dizziness, Bradycardia, Depression, Fatigue/weakness, Bronchospasm with hx of asthma. Check apical and hold if <50, don't stop abruptly, take with meals
Nitrates Relieves angina and increased coronary flow. Hypotension, Headache, Dizziness, Tachycardia. Every 5 minutes 3 doses
Digoxin .5 - 2 is therapeutic level
Baclofen Anti-spastic. Drowsiness, Dizziness, Fatigue, Nausea.Give with food, Avoid alcohol, Do not stop abruptly
Carbamazepine Anticonvulsant. Drowsiness, Dizziness, Rash, Dry mouth, N/V , Depression, Blood dyscrasias(increased WBCs), Hepatotoxicity. Don't stop abruptly, monitor LFT, oral hygiene
Levodopa/Carbidopa Parkinsons. Involuntary movements(dyskinesia), N/V, Hypotension, Hepatotoxicity, Dry mouth. Monitor CBC, LFT, and renal function. Muscle twitching and facial grimacing seen with overdose
Mannitol Increased ICP. Use filter. Fluid def and hypernatremia. Check bag for crystals. Monitor urine, daily weight, CVP, BUN and creatinine.
Pyridostigmine Myasthenia Gravis. Bronchospasm, Bradycardia, Abdominal cramping, Seizures. Give same time every day, atropine is antidote
, Phenytoin, Valproic Acid,Carbamazepine level Phenytoin therapeutic level 10-20 mcg/mL Valproic acid thera. level 50-100 mcg/mL Carbamazepine thera. level 4-12 mg/mL
Albuterol rescue inhaler. Nervousness, Tremor, Chest pain, Palpitations, Arrhythmias. Use spacer if possible, after 5 mins use steroid
Dexamethasone Glucocort. Cough, Rhinitis, Dry mouth, Oral fungal infections. Taper slowly, monitor weight and electrolytes, FSBS, should wear medic alert, suppresses immunity, rinse mouth to prevent thrush
Diphenhydramine For allergies. Anticholinergic effects, drowsy, hypotension. Increase fluids
Rifampin Manages TB. Reddish orange fluids like urine, tears, feces, perspiration. Monitor LFT, no alc, empty stomach if possible
