Cluster A: eccentric disorders Paranoid, Schizoid, Schizotypal
Paranoid Personality disorder longstanding distrust and suspicion of others, unwillingness to forgive and projection of feelings. Social anxiety in childhood. Jealous and controlling as adults.
Paranoid personality disorder tx Simple, direct language, avoid being overly friendly
Schizoid personality disorder Loners, poor academic performance, expressionless, loners that prefer to be alone, sx appear in childhood and adolescence
Schizoid personality disorder tx Do not try to increase socialization, 2nd gen antipsychotics
Schizotypal personality disorder Severe social withdrawal, more so than schizoid. Odd beliefs, rambling conversation, brief intermittent episodes of hallucination or delusion.
Schizotypal personality disorder tx Respect need for social isolation, low dose antipsychotics
Cluster B: Erratic Borderline, Narcissistic, Histrionic, and Antisocial
Borderline personality disorder High suicide and mortality rates, most will attempt suicide. Participates in splitting. paranoid, rage, impulsive
Narcissistic personality disorder Lack of empathy, tendency to exploit others, exaggerated self importance, weak self esteem and hypersensitivity to criticism.
Narcissistic personality disorder tx Lithium for mood swings, CBT, promote self identity
Histrionic personality disorder Excitable, dramatic, attention seeking. Tx is psychotherapy and model concrete language
Antisocial personality disorder Profound lack of empathy and absence of remorse or guilt. Sx peak in mid teens to 20s
Cluster C Anxious Avoidant, dependent, obsessive compulsive
Avoidant personality disorder Decreased functioning in social situations, preoccupied with rejection, humiliation, and failure. S
Dependent personality disorder High need to be taken care of, fear of separation, tx with psychotherapy
Obsessive compulsive personality disorder Limited emotional expressions, indecisiveness, doesn't like change. Constant rehearsal of socil responses.
OCPD tx Clomipramine or fluoxetine for obsessions, anxiety, and depression.
