Fear or avoidance of situations in which escape may be difficult (or embarrassing) or help might not be available if they have panic symptoms. Will avoid movie theaters, grocery stores etc. Has to be multiple places the person is avoiding.--> 1.3% agoraphobia
Excessive or unreasonable(worry excessively about money even when you have plenty) worry that is difficult to control. They worry about everything. Is a dominant feature of their day and it must be all consuming.--> 5.7% Generalized anxiety disorder (GAD)
Fear of negative evaluation in social situations and causes impairment. Fear of any situation where another person is present and fearful that they are evaluating you. Ex.drop class you need to graduate so you don't have to present. -->12.1% Social Anxiety disorder
Lack of speech in one or more settings in which speech is socially expected.--> 0.5% Selective mutism
Fear of harm to self or attachment figure that will cause separation-causes impairment to parent as well because of attachment issues. Think "something bad will happen if I am separated from my parents. -->4.1% Separation anxiety disorder
Specific object or situation (blood-injection-injury, situational, environment, animal). People with blood-injection-injury might start to refuse preventative care. -->12.5% Specific phobia
Traumatic event followed by the set of intrusion, avoidance, cognitive/mood, and arousal/reactivity symptoms. Need to have a certain amount of each symptom. Can get from seeing the traumatic event to a loved one. Posttraumatic stress disorder (PTSD)
Similar to PTSD but occurs within the 1st month after trauma. Acute Stress disorder
Intense longing for or preoccupation with a person who died 1+ years ago. Something is preventing you from moving on. Prolonged Grief Disorder
Milder anxiety or depressive reaction to life stress. Something specific happened in someone's life, have anxiety or depression from it, causes impairment. Adjustment Disorder
Before age 5, unable or unwilling to forms attachments with caregivers. Almost always associated with early severe stress (abuse) or neglect. Attachment disorders
Obsessions and/or compulsions that are time consuming or impairing. Having the thought that something bad is going to happen -therefore doing something to stop the anxiety. Obsessive-compulsive disorder
Preoccupation with an imagined defect in appearance. One of the highest rates of suicide. Body dysmorphic disorder
Persistent difficulty discarding or parting with possessions, regardless of actual value. Really tight attachment to seemingly meaningless things. Hoarding disorder
Hair pulling disorder-get bald spots, driven by anxiety and doing things to make the anxiety feel better. Trichotillomania
Skin picking disorder-can get very severe, is also driven by anxiety and doing something to stop it. Excoriation
Obsession-Needing things to be symmetrical/aligned just so, urges to do things over and over until they feel "just right" Compulsions-putting things in certain order, repeating rituals Symmetry/exactness/"just right" subtype
Obsession-germs, fear of germs or contaminants compulsions-repetitive or excessive washing, using gloves, masks to do daily tasks cleaning/contamination subtype
Obsessions-Fears, urges to harm self or others, fear of offending God. Compulsions-Checking, avoidance, repeated requests for reassurance Forbidden thoughts or actions (aggressive/sexual/religious) subtype
Comorbidity of anxiety disorders and anxiety with other disorders 55%-76% (but need careful differential diagnosis)
Suicide attempt rates? really high in panic disorders and highest in body dysmorphic disorder
Biological vulnerability-genetic influences on heightened amygdala or HPA axis reactivity, behavioral inhibition Specific psychological vulnerability-anxiety sensitivity Generalized psychological vulnerability-low perceived self-competence etc. Three vulnerabilities that contribute to anxiety and related disorders
Temperament trait characterized by a hesitancy to interact with novel people and situations. Stable trait that increases risk for anxiety disorders, particularly social anxiety disorder. behavioral inhibition
Change what we think which affects how we feel and act. Then change behaviors-can focus on behaviors and trying to stop avoidance and everything else will fall into place. Then change how we fell to change how we think and act. CBT
OCD-end of treatment-45%, follow up-44% Social anxiety disorder-end of treatment-40%, follow up-45% panic disorder-end of treatment-55%, follow up-57% GAD-End of treatment-56%, follow up-65% PTSD-End of treatment-63%, follow up-63% percentage of patients no longer meeting criteria following treatment.
SSRI's - most effective also use benzodiazepines but these are terrible meds combined with CBT interfere with the CBT because you need to feel the spike of the anxiety and the later decrease. Psychopharmacological treatments
