Dorsum The top of the hand or foot
Plantar side of the food
Palmar palm of the hand
Review of Systems (ROS) Description of the individual in order to discover any symptoms not directly related to the main problem
Social History A record of habits like smoking, drinking, drug abuse, and sexual practices that can impact health.
Coronal Divides the body along a hypothetical plane from front to back
Transverse Divides body from top to bottom
Prone Lying down on belly
Supine Lying down on back
Proximal Closer to the center
Degeneration To be getting worse
Saggital Divides the body along a hypothetical plane from left to right
Lateral Out to the side
Ascultation To listen
Mortality The risk for dying
Morbidity The risk for being sick
Caudal Toward the bottom
Ventral/antral/anterior The front
Impression Another word way of saying assessment
Differential Diagnosis A list of conditions the patient may have based on the symptoms exhibited and the results of the exam
Prophylaxis Preventative treatment
Palliative Treating the symptoms but not getting rid of the cause
Disposition What happened to the patient at the end of the visit often used at the end of the ED notes to reference where the patient went after the visit.
Distal Farther away from the center
Unremarkable Another way of saying normal
Alert Able to answer questions; responsive; interactive
Noncontributory Not related to specific problem
Afebrile To not have a fever
Exacerbation It is getting worse
Sequlea A problem resulting from a disease or injury
