Dulaglutide(Trulicity), How often is it taken, can you take it with other oral antidiabetics, does it increase risk of low blood glucose? Taken once a week, can be taken with oral antidiabetics, increases risk of low blood glucose
Dulaglutide(Trulicity) complications Pancreatitis, thyroid tumors, kidney failure, vison changes, hold dose 1 week before surgery
Trulicity basic pt ed Once weekly sub Q, refrigerate, and discard every 30 days. Nausea usually subsides after 2 weeks
Trulicity evaluation Preprandial glucose levels 90-130 mg/dL, post prandial glucose levels <180 mg/dL
Metformin MOA, ages used, can it cause hypo? what else does it decrease, and when should you hold Decreases glucose production in the liver, reduces insulin resistance. Used 10 years or older, and can not cause hypoglycemia. Decreases LDLs, hold for 48 hrs after receiving IV contrast because it can cause lactic acidosis. Off label use PCOS
Empaglifozin MOA, contraindications, SE, black box warning Block resorption of glucose at the proximal convoluted tubule. Reduces blood pressure, promotes weight loss, decreases risk of cardiovascular death. Black box warning - higher risk for lower limb amputations
Glipizide Stimulates pancreatic beta cells to release insulin, may improve insulin sensitivity over time. may decrease effects of thiazide diuretics. Discontinue 1 month before delivery to reduce risk of neonatal hypoglycemia.
Side effects of glipizide Hypoglycemia, N/V, weight gain, photosensitivity, blurred vision, liver enzymes, cholestatic jaundice
Sitagliptin (Januvia) Augments incretin, promoting release of insulin and decreases secretion of glucagon.
Complications of sitagliptin Usually well tolerated, but may cause pancreatitis rarely
Sx of sitagliptin Headache, sore throat, body aches, stuffy or runny nose.
Sitagliptin administration Once daily with or without food
