Direct contact vs indirect contact? which one is worse? direct- focal point, fall, blow, penetrating injury indirect- inertia, whiplash, jolts, shaken baby syndrome, WORSE
ways to categorize TBI? MOI-direct/indirect , Location-focal/diffuse , Severity-GCS
what is the most common type of TBI? closed skull injury
what is the description for the dura, arachnoid and pia layers? dura- tough arachnoid - weblike pia - delicate touches brain
where is CSF made? what is CSF function? plexuses in ventricles nutrients, waste removal, protection
epidural bleed? Lucid interval arterial bleed Lens shape on CT Bakinski sign blown pupil on injury side
Subdural bleed? one side weakness, Cresent shaped on CT needs monitoring Headache progressively gets worse Caused by acceleration, deceleration injury
Intracerebral hemorrhage? Projectile vomiting sudden severe symptoms Ipsilateral keep BP at 140 or less, EVD to drain, Stereotactic device with burr hole
Bleed risk factors? Hypertension smoking liver disease diabetes NSAIDs Antiplatelets Anticoagulation meds
hydrocephalus is the most common cause of what bleed? subarachnoid- common in babies
what is the most aggressive brain tumor ? glioblastoma - started in brain tissue
Treatment for brain tumors? Treat ICP, chemo/radiation/ Steroids antiseizure meds prophylactically personality changes common
Considerations for patients with brain tumors? neutropenic precautions- low WBC- need antibiotics Thrombocytopenia- Need CBC, Platelets test bleeding starts 150-450k-if low give IG or platelets
