Medication error any preventable event causing or leading to inappropriate medication use and/or harm to the patient.
The 5 Rights A way to inspect a prescription to ensure the safety of the patient, modeled after nursing 7 rights. Right patient, right drug, right strength, right route, right time
Polypharmacy the use of multiple medications
Negligence Failure to do something that a reasonable person might do or doing something that a reasonable person might not do
Malpractice Negligence performed by a professional
Pharynx mucus-lined chamber that connects nose and mouth to larynx and esophagus
Larynx commonly known as the “voice box”
Trachea commonly known as the “windpipe”
Bronchi large tubes that connect the trachea to the lungs
Bronchioles smaller branches of tubes throughout the lungs
Alveoli sacs in the lungs that inflate during inhalation
Histamine a chemical which is released by cells in response to injury and in allergic and inflammatory reactions, causing contraction of smooth muscle and dilation of capillaries.
Anaphylaxis life-threatening allergic reaction, involves the entire body
Leukotrienes substance in the body similar to histamine, related to allergies and inflammation. Not as powerful, but longer lasting than histamines
Glucocorticoids another term for corticosteroids
Asthma a respiratory condition marked by spasms in the bronchi of the lungs, causing difficulty in breathing. An asthma attack usually results from an allergic reaction or strenuous exercise.
Bronchitis inflammation of bronchi and bronchioles
Pneumonia inflammation of the lungs caused by bacterial or viral infection
Usual & Customary Price Price the provider most frequently charges the general public for the same drug
Maximum Allowable Cost The greatest unit price that the payer or pharmacy benefit manager (PBM) will pay
Average Wholesale Price The average price at which a wholesaler sells drugs to pharmacies, physicians, and other consumers
Compounding In pharmacy, the process of combining active and inactive ingredients to make a medication, reconstituting drugs is NOT considered compounding.
Dispensing fee Fee for a pharmacy’s professional services, determined by several factors specific to a pharmacy, including components of pharmacy operating costs such as packaging, tech labor, and overhead
Drug Utilization Review a cost saving and patient safety measure requiring pharmacy technician or pharmacist to inspect patient’s prescription to ensure usage is appropriate.
