| Term | Definition |

| Therapeutic Duplication | a drug prescribed to a patient that is either in the same class or the has a similar therapeutic effect as a medication the patient is already taking |
| Hazardous Drug | drugs that, if exposed to, can cause acute chronic health effects, adverse reproductive outcomes or cancer. |
| High Alert Medication | Drugs that when used in error can cause significant patient harm. They include all opiates, oral anti-diabetic medications, insulin, anti coagulants, potassium chloride, and sodium chloride above concentration of 0.9% |
| Narrow Therapeutic Index | Drugs where small differences in dose or concentration in the blood can cause serious adverse effects for the patient. These medications include: anti-seizure medications, cardiac medications, anticoagulants, theophylline, and thyroid hormone replacement |
| Look alike-sound alike drugs | Medications with similarly spelled or similar sounding either generic or brand names that can be confused for each other (ex. clozapine and clonidine) |
| Tall man letters | technique to differentiate commonly confused medications, implemented by the Institute for Safe Medication Practices. The different letters of look alike/sound alike medications are capitalized to emphasis the difference (ex. cloZAPINE, cloNIDINE) |
| USP 797 | section of the United States Pharmacopeia that sets standards for sterile compounding |
| USP 795 | section of the United States Pharmacopeia that sets standards for non-sterile compounding |
| USP 800 | section of the United States Pharmacopeia that sets standards for the ordering, receiving, handling, compounding, and dispensing of hazardous drugs. |
| Substance abuse | The use of illegal drugs or the use of prescription or over-the-counter drugs or alcohol for purposes other than those for which they are meant to be used |
| Habituation | a state of psychological dependence on a drug, characterized by a desire to continue use to feel "normal" |
| Tolerance | When the body requires a higher dose of a drug to produce the same effect that a lower dose once provided |
| Withdrawal | Physical and mental symptoms that a person has when they suddenly stop or cut back the use of an addictive substance, some can be life-threatening |
| Overdose | when a toxic amount of a drug, or combination of drugs overwhelms the body |
| Delirium tremens | a severe, potentially life-threatening form of alcohol withdrawal that occurs in people with long-term, heavy alcohol use after they stop drinking |
| Substance Use Disorder | preferred term for conditions previously labeled as addiction and substance abuse |
| Geriatric patients | patients aged 65 or older |
| Polypharmacy | multiple medications prescribed simultaneously to the same patient |
| Beers Criteria | A list of medications published by the American Geriatric Society that should be avoided or used cautiously in elderly patients |
| RSV | Respiratory Syncytial Virus |
| SUD | Substance Use Disorder (preferred term for "addiction") |
| DT | delirium tremens |
| UTI | urinary tract infection |
